Death anxiety in patients with primary brain tumor: Measurement, prevalence, and determinants

Objective
This study investigated death anxiety in patients with primary brain tumor (PBT). We examined the psychometric properties of two validated death anxiety measures and determined the prevalence and possible determinants of death anxiety in this often-overlooked population.
MethodsTwo cross-sectional studies in neuro-oncology were conducted. In Study 1, 81 patients with PBT completed psychological questionnaires, including the Templer Death Anxiety Scale (DAS). In Study 2, 109 patients with PBT completed similar questionnaires, including the Death and Dying Distress Scale (DADDS). Medical and disease-specific variables were collected across participants in both studies. Psychometric properties, including construct validity, internal consistency, and concurrent validity, were investigated. Levels of distress were analyzed using frequencies, and determinants of death anxiety were identified using logistic regression.
ResultsThe DADDS was more psychometrically sound than the DAS in patients with PBT. Overall, 66% of PBT patients endorsed at least one symptom of distress about death and dying, with 48% experiencing moderate-severe death anxiety. Generalized anxiety symptoms and the fear of recurrence significantly predicted death anxiety.
Significance of resultsThe DADDS is a more appropriate instrument than the DAS to assess death anxiety in neuro-oncology. The proportion of patients with PBT who experience death anxiety appears to be higher than in other advanced cancer populations. Death anxiety is a highly distressing symptom, especially when coupled with generalized anxiety and fears of disease progression, which appears to be the case in patients with PBT. Our findings call for routine monitoring and the treatment of death anxiety in neuro-oncology.

Introduction
The long-term management of psychiatric wounded patients with prolonged disorders requires a rethinking of our practice of care.
ObjectivesThe aim is to propose an integrative model of all valid therapies in the post-traumatic-stress disorder while taking care of comorbidities and ensuring patient support in the different administrative procedures that permit reconstruction. Repeated short-term hospitalizations can meet this objective by mobilizing resources, creating group dynamics, restoring a space of safety, allowing a rupture with the environment, preventing recurrence of crises, and by encouraging the histicization of trauma by the temporal sequences of intra/extra-hospitalisation repetition.
MethodWe propose, by means of a review of the literature, to discuss on a psychopathological level the interest and limits of this mode of care.
ResultsThis work reveals the specific therapeutic effects of repeated programmed hospitalizations, which constitute a new modality of institutional psychotherapy.
ConclusionRethinking the place of hospitalisation in the management of psychiatric illnesses can be useful to all psychiatrists who follow patients with chronic and co-morbid disease.

Impact of conservation measures on demography and genetic variability of livestock breeds

Conservation of animal genetic resources requires regular monitoring and interventions to maintain population size and manage genetic variability. This study uses genealogical information to evaluate the impact of conservation measures in Europe, using (i) data from the Domestic Animal Diversity Information System (DAD-IS) and (ii) a posteriori assessment of the impact of various conservation measures on the genetic variability of 17 at-risk breeds with a wide range of interventions. Analysis of data from DAD-IS showed that 68% of national breed populations reported to receive financial support showed increasing demographic trends, v. 51% for those that did not. The majority of the 17 at-risk breeds have increased their numbers of registered animals over the last 20 years, but the changes in genetic variability per breed have not always matched the trend in population size. These differences in trends observed in the different metrics might be explained by the tensions between interventions to maintain genetic variability, and development initiatives which lead to intensification of selection.

Treatment options for Hepatitis C infection have greatly improved with direct-acting antiviral (DAA) combinations achieving high cure rates. Nevertheless, the cost of this treatment is still high and access to treatment in many countries has been preferentially reserved for patients with more severe fibrosis (F3 and F4). In this French nationwide study, we investigated the epidemiological characteristics and genotype distribution of hepatitis C virus (HCV) in treatment-naive patients with METAVIR fibrosis stages between F0 and F2 in order to identify patient profiles that became eligible for unrestricted treatment in a second period. Between 2015 and 2016 we collected data from nine French university hospitals on a total of 584 HCV positive patients with absent, mild or moderate liver fibrosis. The most represented genotypes were genotype 1b (159/584; 27.2%), followed by genotype 1a (150/584; 25.7%); genotype 3 (87/584: 14.9%); genotype 4 (80/584; 13.7%). Among genotype 4: 4a was predominantly encountered with 22 patients (27.5% of genotype 4). Genotypes 1b and 1a are currently the most frequent virus types present in treatment-naive patients with mild fibrosis in France. They can be readily cured using the available DAA. Nevertheless, non-a/non-d genotype 4 is also frequent in this population and clinical data on the efficacy of DAA on these subtypes is missing. The GEMHEP is the French group for study and evaluation of viral hepatitis on a national scale. Data collection on epidemiological and molecular aspects of viral hepatitis is performed on a regular basis in all main French teaching hospitals and serves as a basis for surveillance of these infections. Analysis and trends are regularly published on behalf of the GEMHEP group. Data collection was performed retrospectively over the 2015–2016 period, covering nine main university hospitals in France. A total of 584 hepatitis C positive patients were included in this study. Genotyping of the circulating viruses showed a high prevalence of genotypes 1b and 1a in our population. The epidemiology of hepatitis C is slowly changing in France, particularly as a consequence of the rise of ‘non-a non-d’ genotype 4 viruses mainly originating from African populations. More data concerning treatment efficacy of these genotypes is needed in order to guide clinical care.
Perception of livestock ecosystem services in grazing areas

This study investigates how the ecosystem services (ES) linked to livestock grazing are perceived across countries. A total of 82 case studies collected from 42 countries via survey (53.7% cases from Europe and 46.3% from outside of Europe) have been analysed through a multivariate approach. In all, 18 non-provisioning ES were considered. Overall, the reported impacts of livestock grazing on the different ES were much more positive than negative. Notably, a large proportion of respondents reported either positive or very positive impacts for some cultural ES, namely cultural, historic and natural heritage (84%), knowledge systems and educational values (77%), landscape values (74%), and for some supporting and regulating ES, namely habitat provision (66%), nutrient cycling (65%), and bush encroachment/fire control (66%). Based on multiple regression analysis, geographic origin, stakeholder type and species category, as well as protection status of the grazing area, had significant effects on the perception of the impacts. Respondents reported those impacts as more positive in Europe, in protected areas and where several species were present in the grazing area. A significantly larger proportion of respondents reported recognition of ES provided by the grazing livestock population in European countries (40.9%) compared with non-European countries (23.7%). Based on the survey responses it appears that in non-European countries absence of formal recognition, especially by policy makers, is a major challenge for the continued provision of ES in grazing systems. In Europe, where such recognition is already often included in legislation, the long-term sustainability of related policies and incentives to provide such services is viewed as a major issue by the respondents.

Background
In early 2015, a patient from a cluster of cases of Ebola Virus Disease (EVD) in Monrovia, Liberia traveled to a rural village in Margibi County, potentially exposing numerous persons. The patient died in the village and post-mortem testing confirmed Ebola Virus infection.
ProblemThe Margibi County Health Team (CHT; Kakata, Margibi, Liberia) needed to prevent further transmission of EVD within and outside of the affected villages, and they needed to better understand the factors that support or impede compliance with measures to stop the spread of EVD.
MethodsIn February-March 2015, the Margibi CHT instituted a 21-day quarantine and active monitoring for two villages where the patient had contact with numerous residents, and a 21-day active monitoring for five other villages where the patient had possible contact with an unknown number of persons. One contact developed EVD and quarantine was extended an additional 12 days in one village. In April 2015, the Margibi CHT conducted a household-based EVD knowledge, attitudes, and practices (KAP) survey of the seven villages. From April 24-29, 2015, interview teams approached every household in the seven villages and collected information on demographics, knowledge of EVD, attitudes about quarantine to prevent the spread of EVD, and their quarantine experiences and practices. Descriptive statistics were calculated.
ResultsOne hundred fifteen interviews were conducted, representing the majority of the households in the seven villages. Most (99%) correctly identified touching an infected person’s body fluids and contact with the body of someone who has died from EVD as transmission routes. However, interviewees sometimes incorrectly identified mosquito bites (58%) and airborne spread (32%) as routes of EVD transmission, and 72% incorrectly identified the longest EVD incubation period as ≤seven days. Eight of 16 households in the two quarantined villages (50%) reported times when there was not enough water or food during quarantine. Nine of 16 (56%) reported that a household member had illnesses or injuries during quarantine; of these, all (100%) obtained care from a clinic, hospital, or Ebola treatment unit (ETU).
ConclusionResidents’ knowledge of EVD transmission routes and incubation period were suboptimal. Public health authorities should consider assessing residents’ understanding of Ebola transmission routes and effectively educate them to ensure correct understanding. Quarantined residents should be provided with sufficient food, water, and access to medical care.

Controlled irrigation and nitrogen, phosphorous and potassium fertilization affect the biochemical composition and quality of Arabica coffee beans

Controlled irrigation during the dry period associated with adequate nitrogen (N), phosphorous (P) and potassium (K) fertilization led to the accumulation of biochemical compounds in coffee beans considered as positive precursors of beverage quality. Adult plants of coffee (Coffea arabica ‘Rubi’) were cultivated using different water regimes (WR) and fertilization conditions under the dry climate of the Brazilian Cerrado. Coffee-bean physical characteristics were evaluated as well as biochemical composition by near-infrared analysis. The K treatment mostly affected bean biochemistry, lipid and chlorogenic acid (CGA) contents, which increased with increasing amounts of fertilizer. Caffeine contents increased with higher amounts of N, but no significant effects of P treatment on bean biochemical composition were observed. Sucrose and total lipid contents always appeared higher in beans of non-irrigated plants than those beans from plants grown with continuous irrigation. In contrast, caffeine and CGA contents were higher in beans of irrigated as compared with non-irrigated plants. For the first time, the current results showed that controlled management of irrigation during the dry period associated with reasonable NPK fertilization led to the accumulation of biochemical compounds in coffee beans considered as positive precursors of beverage quality.

Crossbreeding, considering either terminal or rotational crossing, synthetic breed creation or breed replacement, is often promoted as an efficient strategy to increase farmers’ income through the improvement of productivity of local livestock in developing countries. Sustainability of crossbreeding is however frequently challenged by constraints such as poor adaptation to the local environment or lack of logistic support. In this review, we investigate factors that may influence the long-term success or the failure of crossbreeding programs, based on the scientific literature and country reports submitted for The Second Report on the State of the World’s Animal Genetic Resources for Food and Agriculture. Crossbreeding activities vary widely across species and countries. Its sustainability is dependent on different prerequisites such as continual access to adequate breeding stock (especially after the end of externally funded crossbreeding projects), the opportunity of improved livestock to express their genetic potential (e.g. through providing proper inputs) and integration within a reliable market chain. As formal crossbreeding programs are often associated with adoption of other technologies, they can be a catalyst for innovation and development for smallholders. Given the increasing global demand for animal products, as well as the potential environmental consequences of climate change, there is a need for practical research to improve the implementation of long-term crossbreeding programs in developing countries.
Assessing the risk status of livestock breeds: a multi-indicator method applied to 178 French local breeds belonging to ten species

Breed risk status assessment methods are key components of country-based early warning and response systems. In this study, a multi-indicator method was developed to assess the risk status of livestock populations. Six indicators were used: (i) the current number of breeding females; (ii) the change in the number of breeding females over the last 5 years or generations (depending on the species); (iii) percentage of cross-breeding; (iv) effective population size; (v) breeders organization and technical support; and (vi) socio-economic context. To make these indicators comparable, observed values were converted into scores on a six-point scale (from 0 = no threat to 5 = maximum threat); a specific conversion method was used for each indicator. For each breed, the different scores were analysed graphically and an overall score was calculated by averaging the six separate indicator scores. This approach was applied to 178 French local breeds, belonging to ten different species: horse, donkey, goat, pig, chicken, turkey, goose and Pekin duck. A large percentage of local breeds were found to be at risk to be lost for farming, although the results were species dependent. All local equine and pig breeds, as well as almost all local poultry breeds appeared to be endangered. About 80 percent of local goat and cattle breeds, and half local sheep breeds were also found to be at risk. The usefulness of this method with regards to conservation strategies and public policy is discussed.

Summary
Introduction
Foundation species represent excellent model systems for understanding the broad consequences of variation on community and ecosystem processes as they provide a focal resource upon which associated interacting species depend. As foundation species (Dayton 1972; Ellison et al. 2005), trees and other dominant plants often create stable conditions via plant traits that allow dependent communities to assemble regularly and influence ecosystem processes such as net primary productivity (NPP) and soil fertility (i.e., nutrient cycling, via accumulations of leaf or root organic matter or root exudates; Zinke 1962; Zak et al. 1986; Binkley and Giardina 1998; Bartelt-Ryser et al. 2005; Wardle 2006). Recent studies in both terrestrial and aquatic habitats have shown that intraspecific genetic variation (defined at multiple genetic scales, including introgression [movement of genes from one species to another], genotypic diversity [studies manipulating the number of genotypes in a population] and genotypic variation [variation among genotypes]) in foundation plants can have community-wide consequences. Intraspecific variation affects associated vertebrate, arthropod and microbial community composition or activity and ecosystem level processes (recently reviewed in Johnson and Stinchcombe 2007; Hughes et al. 2008; Whitham et al. 2008; Bailey et al. 2009). For example, genetic variation resulting from the introgression of genes from one species to another through the process of hybridization has been shown to have important consequences for associated species, communities and ecosystem processes in multiple hybridizing plant species, including Salix spp., Eucalyptus spp., Quercus spp. and Populus spp. (Fritz et al. 1994; Dungey et al. 2000; Hochwender and Fritz 2004; Ito and Ozaki 2005; Wimp et al. 2005; Tovar-Sanchez and Oyama 2006; Bangert et al. 2008). In the Populus system specifically, recent field and common garden studies have shown that genetic variation across a hybridizing system (P. fremontii, P. angustifolia and their natural F1 and backcross hybrids) results in shifts in plant traits, including secondary chemistry, plant water use and above- and belowground productivity (Fischer et al. 2004; Rehill et al. 2006; Schweitzer et al. 2008a; Lojewski et al. 2009). Whether due directly or indirectly to these plant traits, rates of leaf litter decomposition, total belowground carbon (C) allocation and pools of soil nitrogen (N) and rates of net N mineralization also shift along this genetic gradient (Schweitzer et al. 2004, 2008, b; LeRoy et al. 2006; Whitham et al. 2006; Lojewski et al. 2009; Fischer et al. 2007, 2010).
An Updated View of Giant Molecular Clouds, Gas Flows and Star Formation in M51 with PAWS

We present an overview of the latest results from the PdBI Arcsecond Whirlpool Survey (PAWS, PI: E. Schinnerer), which has mapped CO(1-0) emission in the nearby grand-design spiral galaxy M51 at 40pc resolution. Our data are sensitive to GMCs above 105 M⊙, allowing the construction of the largest GMC catalog to date – containing over 1500 objects – using the CPROPS algorithm (Rosolowsky & Leroy 2006). In the inner disk of M51, the properties of the CO emission show significant variation that can be linked to the dynamical environment in which the molecular gas is located. We find that dynamically distinct regions host clouds with different properties and exhibit different GMC mass spectra, as well as distinct patterns of star formation. To understand how this sensitivity to environment emerges, we consider the role of pressure on GMC stabilization (including shear and star formation feedback-driven turbulence). We suggest that, in the presence of significant external pressure, streaming motions driven by the spiral arm can act to reduce the surface pressure on clouds. The resulting stabilization impacts the global pattern of star formation and can account for the observed non-monotonic radial dependence of the gas depletion time. Our findings have implications for the observed scatter in the standard GMC relations and extragalactic star formation laws.

Introduction
Relatively little is understood about the extent to which evolution in one species can result in changes to associated communities and ecosystems, the potential mechanisms responsible for those changes (genetic drift, gene flow or natural selection), the phenotypes or candidate genes that may link ecological and evolutionary dynamics, or the role of rapid evolution and feedbacks. However, linking genes and ecosystems in this manner is fundamental to placing community structure and ecosystem function in an evolutionary framework. This is not an easy endeavour as the field of community genetics is multi-disciplinary (Whitham et al., 2006), and ecological and evolutionary dynamics occur at different spatial and temporal scales. Recent reviews show that plant genetic variation can have extended consequences at the community and ecosystem level (extended phenotype; Whitham et al., 2003) affecting arthropod diversity, soil microbial communities, trophic interactions, carbon dynamics and soil nitrogen availability (Whitham et al., 2006; Johnson & Stinchcombe, 2007; Hughes et al., 2008; Bailey et al., 2009a). Its effects are not limited to single systems or even foundation species, but are common across broadly distributed plant and animal systems, and can have effects at the community and ecosystem level of similar magnitude to traditional ecological factors, such as differences among species (Bailey et al., 2009a, b).
Theory in the fields of community genetics (Shuster et al., 2006; Whitham et al., 2006) and co-evolution (Thompson, 2005) also supports the connection between evolutionary and ecological dynamics (Johnson et al., 2009). Multiple investigators argue that community and ecosystem phenotypes represent complex traits related to variation in the fitness consequences of inter-specific indirect genetic effects (IIGEs) (Thompson, 2005; Shuster et al., 2006; Whitham et al., 2006; Tetard-Jones et al., 2007). In their most basic form, IIGEs occur when the genotype of one individual affects the phenotype and fitness of an associated individual of a different species (Moore et al.,1997; Agrawal et al., 2001; Shuster et al., 2006; Wade, 2007). Such interactions are important in the geographic mosaic theory of co-evolution (Thompson, 2005), the development of community heritability (Shuster et al., 2006) and non-additive responses of community structure, biodiversity and ecosystem function (Bailey et al., 2009a). Empirical evidence for the effects of plant genetic variation on communities and ecosystems, paired with growing theoretical models explaining evolutionary mechanisms for these results, provides a solid foundation for understanding how evolutionary processes, such as drift and selection, may affect community structure and ecosystem function.

The East Flanders Prospective Twin Survey (EFPTS) is a prospective, population-based registry of multiple births in the province of East Flanders, Belgium. EFPTS has several unique features: it is population based and prospective, with the possibility of long-term follow-up; the twins (and higher order multiple births) are ascertained at birth; basic perinatal data recorded; chorion type and zygosity established; and since 1969 placental biopsies have been taken and frozen at −20 °C for later determination of genetic markers. The EFPTS is the only large register that includes placental data and allows differentiation of 3 subtypes of monozygotic (MZ) twins based on the time of the initial zygotic division: the dichorionic–diamnionic pairs (early, before the 4th day after fertilization), the monochorionic–diamnionic pairs (intermediate, between the 4th and the 7th day post fertilization), and the monochorionic–monoamnionic pairs (late, after the 8-day post fertilization). This added a new dimension to didymology (the science of twins; didymos is the Greek word for twin): the timing of MZ twinning. Studies can be initiated taking into account primary biases, those originating in utero. Such studies could throw new light on the controversy over the validity of the classic twin method, the consequences of early embryological events (before and just after implantation of the embryo), the origin of congenital malformations, the sex proportion of multiples, the gene–environment interactions as far as intrauterine environment is concerned, to name but a few.
Sinonasal mucosal melanoma: retrospective survival study of 25 patients

Objective:
To determine potential prognostic factors for survival in patients with mucosal malignant melanoma of the sinonasal tract.
Methods:Patients managed between 1991 and 2008 were assessed retrospectively. The seventh edition Union for International Cancer Control (7th UICC) tumour-node-metastasis classification was used for tumour staging. Kaplan–Meier and log rank tests were used for survival analysis.
Results:Twenty-five patients were studied (six were tumour stage three, eight tumour stage four(a) and 11 tumour stage four(b)). Surgery was performed on 23 patients (92 per cent). Fifteen received post-operative radiotherapy. Mean follow up was 31.3 months (range, two to 99 months). Three-year disease-free survival was improved in patients with stage four tumour arising from the nasal fossa, versus other sites, and in those with stage four tumour treated with surgery plus adjuvant radiotherapy, versus other treatments.
Conclusion:Patients with melanoma of the nasal cavity have very poor survival rates. Treatment is still based on adequate surgical resection with safe margins. In this study, post-operative radiotherapy improved local control only for stage four tumours.
Resistance and 1/f noise between circular contacts on conductive thin films

There is a need to characterize the quality of contacts and the noise properties of new materials deposited or grown as thin films. Poor contacts are interface dominated. Perfect contacts have a negligible interface contribution and there is only a resistance and noise contribution from outside the contact region. The presence of current crowding enhances the resistance and noise contribution. Such contacts are called constriction dominated contacts. The conductive film is characterized by its sheet resistance and normalized conductance fluctuations for a unit surface. The resistance and noise is studied between two circular top electrodes of the same diameter on the conductive. To distinguish between perfect and poor contacts and to characterize the thin film in case of good contacts, we need a set of contacts with different diameters. Models for perfect and poor contacts are investigated. The scaling of resistance and noise with contact radius r is for interface dominated poor contacts: Ri$\propto$ 1/r2 and SRi$\propto$ 1/r6. In contrast, perfect contacts with contact diameter (2r) much smaller than the distance between the centers (2b) show: Rc$\propto$ ln(b/r) and SRc$\propto$ 1/r2. From the resistance and noise measurements between constriction dominated perfect contacts, the sheet resistance and normalized noise of the thin film are calculated.

Collaboration is used by the US National Security Council as a means to integrate inter-federal government agencies during planning and execution of common goals towards unified, national security. The concept of collaboration has benefits in the healthcare system by building trust, sharing resources, and reducing costs. The current terrorist threats have made collaborative medical training between military and civilian agencies crucial.
This review summarizes the long and rich history of collaboration between civilians and the military in various countries and provides support for the continuation and improvement of collaborative efforts. Through collaboration, advances in the treatment of injuries have been realized, deaths have been reduced, and significant strides in the betterment of the Emergency Medical System have been achieved. This review promotes collaborative medical training between military and civilian medical professionals and provides recommendations for the future based on medical collaboration.

INTRODUCTION
The emerging field of community and ecosystem genetics has so far focused on how the genetic variation in one species can influence the composition of associated communities and ecosystem processes such as decomposition (see definitions in Table 3–1; reviews by Whitham et al. 2003, 2006; Johnson & Stinchcombe 2007; Hughes et al. 2008). A key component of this approach has been an emphasis on understanding how the genetics of foundation plant species influence a much larger community. It is reasoned that because foundation species structure their ecosystems by creating locally stable conditions and provide specific resources for diverse organisms (Dayton 1972; Ellison et al. 2005), the genetics of these species as “community drivers” are most important to understand and most likely to have cascading ecological and evolutionary effects throughout an ecosystem (Whitham et al. 2006). For example, when a foundation species’ genotype influences the relative fitness of other species, it constitutes an indirect genetic interaction (Shuster et al. 2006), and when these interactions change species composition and abundance among individual tree genotypes, they result in individual genotypes having distinct community and ecosystem phenotypes. Thus, in addition to an individual genotype having the “traditional” phenotype that population geneticists typically consider as the expression of a trait at the individual and population level, community geneticists must also consider higher-level phenotypes at the community and ecosystem level. The predictability of phenotypes at levels higher than the population can be quantified as community heritability (i.e., the tendency for related individuals to support similar communities of organisms and ecosystem processes; Whitham et al. 2003, 2006; Shuster et al. 2006).
